ANNEX VI - APPLICATION FORM WITH A VIEW TO THE RECOGNITION, DECLARATION OF ENFORCEABILITY OR ENFORCEMENT OF A DECISION IN MATTERS RELATING TO MAINTENANCE OBLIGATIONS

This form is to be completed if you have an existing maintenance decision that you want to have recognised and enforced in another EU country. The form should be completed in a language accepted by the receiving country. Please be aware that the language of the answers must be the same as the form. It is possible to complete this form on-line and this is the recommended, this allows you to complete the form in your language and convert it into a language accepted by the receiving country. If you wish to complete it by hand please use a pen and capital letters and submit the version of the language accepted by the receiving country.
